REGISTER A DEATH:
Every individual has the responsibility to inform any Death in their house to the Registrar of births and deaths. Information may be given at GVMC Call Centre 1913. Any information regarding a death may also be sent in Death Report Form and submitted to the Registrar of Births and Deaths.
All the hospitals and nursing homes will inform regarding the deaths occuring in their institutions to the Registrar Births and Deaths in the Death Report Form.
This information should be submitted within 21 days of occurrence and a certificate of Death on free of cost under section 12 of the Act will be sent.
Late Registration:
Under section 13.I.II.III of the Birth and Death Registration Act 1969 a Death should be reported before 21 days. The event can be registered within one month on payment of late fee.
Between one month and one year of occurrence of the event the individual should submit an affidavit from a notary along with a payment of late fees for Registration to Registrar of Birth and Deaths.
In case the registration is not done within one year of death. and application shall be made to Registrar, Births and Deaths along with Death application form and a fee of Rs.50.00 paid in any e-Seva centre. This application is may forwarded to the Revenue Divisional Officer (RDO) who will issue an order for registration on enquiry based on the non-availability certificate issued by Registrat, Birth and Deaths, GVMC. The Registrar registers the death on RDO's order and issue the death certificate. |
